摘　　要：目的:探讨经阴道三维彩超检查诊断早期宫颈癌(Ⅰb期~Ⅱa期)的应用价值。方法:对96例早期宫颈癌患者和其他150例患者作经阴道三维彩超检查,并与手术、病理结果对照分析。结果:在96例早期宫颈癌患者中,Ⅰb1期67例,Ⅰb2期18例,Ⅱa期11例,其中鳞癌73例,腺癌19例,腺鳞癌4例。肿块大小:5mm×6mm×9mm^32mm×35mm×46mm。在150例对照组中,子宫颈正常组50例,宫颈肌瘤23例,宫颈粘膜下子宫肌瘤16例,颈管息肉11例,宫颈子宫内膜异位囊肿23例,宫颈纳氏囊肿27例。经阴道三维彩超显示宫颈癌宫颈肿块处血管形态分布可分成4型。早期宫颈癌三维阴道彩超诊断符合率为86.5%。结论:经阴道三维彩超能显示宫颈及宫旁组织的立体图像,宫颈处显示的4种血管形态分布对鉴别宫颈肿瘤的良恶性质有较高价值。Objective：To investigate the clinical value and sonographic features in early-stage cervical cancer by transvaginal three-dimensional（3D）color Doppler ultrasound.Methods： A total of 96 patients with early-stage cervical cancer（Ⅰb stage～IIa stage） and 150 cases in the control groups of transvaginal 3D ultrasonic diagnosis compared with surgical pathology.Results： In between Ⅰb1-stage was 67 cases,Ⅰb2 was 18 cases,Ⅱa was 11 cases.Squamous cell carcinoma was 73 cases.Adenocarcinoma was 19 cases and adenosquamous carcinoma was 4 cases.The tumor sizes were from 5 mm×6 mm×9 mm to 32 mm×35mm×46 mm.Among 150 cases of the control groups,including 50 cases without genital diseases,23 cases of cervical fibroma,16 cases of uterine submucous myoma projecting to cervix,11 cases of cervical polyps,23 cases of endometriosis cyst,27 cases of Naboth cyst.It showed four tapes of vascular distribution in cervical cancer by transvaginal 3D color Doppler ultrasound and power Doppler imaging.The coincidence rate was 86.5% of the early-stage cervical carcinoma by transvaginal 3D ultrasound was accordant to pathologic results.Conclusions： The 3D color Doppler ultrasound by transvaginal is a useful tool in the detection of the early-stage cervical carcinoma.Because it not only showed image of high resolution,but also it showed 4 types of vascular distribution in tumors which is valuable in distinguishing benign and malignant status.
