排卵监测和宫腔内人工授精时机的选择

摘  要:不孕患者30例50个周期阴道B超观察、尿促黄体激素(LH)测定及宫颈粘液(CS)评分联合监测排卵,择机进行宫腔内人工授精(IUI).观察表明:自然排卵周期联合监测可准确地预计排卵时间,IUI24h内排卵率达85%;克罗米酚(CC)促排卵因成熟卵泡大小差异大,且尿LH阳性率低及CS评分不典型,影响了准确掌握IUI时机,IUI24h内排卵率36.4%;CC+绒膜激素(HCG)促排卵联合监测,根据卵泡大小决定HCG给药时间,可较准确地预计排卵时间,IUI24h内排卵率达63.2%.In fifty cycles of 30 cases, ovulation were observated with detecting follciular size and urinary LH as well as cervical score (CS) for IUI. The results showed that ovulation of spontaneous cycles could be predicted with follicular size combined with level of urinary LH and CS, that ovulation rate was 85% in 24 hours after IUI. In CC induced ovulatory cycles, it wasn't easy to predict ovulation because of difference in the size of mature follicle, low level of urinary LH with its peak occured much earlier and atypical CS. The ovulation rate was 36.4% in 24 hours after IUI. In CC+HCG induced ovulatory cycles, the ovulation could be move easily predicted with follicullar size while handed on injecting HCG time. That ovulation rate was 63.2% in 24 hours after IUI.
