RU486合并前列腺素终止早孕的效果及对性激素影响的研究

摘　　要：4O例早孕妇女,停经49天以内,给予RU 486合并前列腺素(Sulprostone)终止早孕。对象随机分为两组(每组20例)。组Ⅰ,口服RU 486 25mg2次/日×4天,在第四天上午肌注Sulprostone 0.25mg;组Ⅱ,RU 486 25mg2次/日×3天,在第三天上午肌注Sulprostone 0.25mg。对象必须从服药的第一天起连续四天(组Ⅱ三天)上午来门诊,此外第8、15及43天亦须来门诊,接受访视及取血,用以测定B-hCG、E_2、P、PRL及Cortisol等。检测结果,血清B-hCG及E_2水平在服药期间继续上升,但流产后迅即下降;PRL及Cortisol在服药期间及服药后,血清水平上下波动不大;P在服药期间水平未见上升,但流产后迅即下降。40例对象,不论服药三天或四天,均发生完全流产,无一例需要刮宫或输血。Healthy pregnant women were given RU 486 and sulprostone after amenorrhea had occurred wirhin 49 days. The patients were allocated randomly to 2 groups (20 subjects per group). Women in group Ⅰ received 25 mg RU 486 twice daily for 4 days plus one single intramuscular injection of 0.25 mg sulprostone on the morning or the fourth RU 486 Treatment day (ie day 4). Women in group Ⅱ received 25 mg RU 486 twice daily for three days plus one single intramuscular injection of 0.25 mg sulprostone on the morning of the third Treatment day (ie day 3). All of the subjects were asked to attend the hospital on each treatment day and day 8, 15, 43 for follow-up visit, and blood samples collected fored hormone analysis of serum β-hCG, E_2, P, PRL and cortisol. Serum β-hCG and E_2 levels of both the groups increased during the first three or four treatment days. After expulsion of concept products, however, there was a sharp decline in β-hCG and E_2 levels. The serum P values during treatment were of no significant. difference between the two groups, but there was a sharp decline after abortion in each case. There was no appreciable change of either the PRL or cortisol levels in the blood samples during and after the treatment. Of all 40 cases the treatment led to complete abortion, none of them needed curettage and blood transfusion.
