理气通便协定方鼻饲加芒硝外敷治疗危重病患者腹腔高压32例的临床观察

摘　　要：目的观察中药理气通便协定方(自制)鼻饲加芒硝外敷对危重病患者腹腔高压治疗效果。方法将62例伴有腹腔高压的危重病患者随机分为治疗组32例和对照组30例,对照组给予常规治疗,治疗组在常规治疗基础上加用鼻饲理气通便协定方联合加芒硝外敷。观察两组患者第3、7、14天腹内压的改变及腹内压降至正常和症状明显缓解的时间,比较两种方法治疗腹腔高压的有效性和安全性。结果治疗组治疗后腹内压下降,第7、14天腹内压较对照组明显下降,差异均有统计学意义(t分别=3.20、4.33,P均<0.05);治疗组的总有效率明显高于对照组,能减少患者住院时间,两组比较差异亦有统计学意义(χ2=7.92,t=4.49,P均<0.05)。治疗组4例病人出现明显腹泻,经对症治疗后好转。结论理气通便协定方鼻饲联合芒硝外敷治疗危重病患者腹腔高压疗效显著,且无明显毒副作用。Objective To investigate the efficacy and safety for the nasal feeding of liqitongbian agreement prescription （home preparation） plus external application of natrii sulfas in critically ill patients. Methods A total of 62 cases of critically ill patients with abdominal hypertension were randomly divided into two groups： 30 cases in the control group which received the conventional treatment; 32 cases in the treatment group which received the convention treatment combined with the nasal feeding of liqitongbian agreement prescription plus natrii sulfas. The intra-abdominal pressure of patients on day 3,7 and 14, and time for symptom alleviation were measured to compare the efficacy and safety between the control and treatment groups. Results The intra-abdominal pressure of the treatment group on day 7 and 14 was significantly decreased compared with that of the control group （t=3.20,4.33,P 0.05）; The total efficacy of the treatment group was greater than that of the control group （χ2=7.92; t=4.49,P0.05）. However, 4 cases of patients in the treatment showed diarrhea which was cured after the treatment. Conclusions Nasal feeding of liqitongbian agreement prescription plus external application of natrii sulfas could remarkably decrease abdominal hypertension and show no obvious side effects.
