经皮颈外静脉建立长期血透导管的临床应用

摘　　要：目的探讨经皮颈外静脉穿刺建立长期血透导管的方法及护理。方法 45例维持性血液透析患者分为观察组20例和对照组25例。观察组采用经皮颈外静脉建立长期血透导管,对照组采用经皮颈内静脉建立长期血透导管。观察比较两组一次穿刺成功率、穿刺过程中的并发症发生率及导管使用过程中的并发症发生率。结果观察组的一次穿刺成功率、穿刺过程中的并发症发生率明显低于对照组(P(0.05),而导管使用过程中的并发症发生率比较差异无统计学意义(P>0.05)。结论经皮颈外静脉建立长期血透导管可以作为维持性血液透析患者建立长期血透导管的又一选择,尤其适合于颈内静脉穿刺失败者、颈内静脉长期导管感染需更换导管者、颈外静脉显露较粗者。Objective To investigate clinical feasibility and nursing of long-term Dacro-cuffed dural lumen catheter through external Jugular for hemodialysis patients.Methods Maintenance hemodialysis patients were divided into experimental group （20 cases） and control group（25 cases）.Experimental group were hemodialysis patients with long-term Dacro-cuffed dural lumen catheter through external Jugular.Control groups were hemodialysis patients with long-term Dacro-cuffed dural lumen catheter through Jugular vein.Success rate of puncture,complication of puncture and using process between the two groups were compared.Results Success rate of puncture was higher and complication of puncture was lower in experimental group when compared with control group（P0.05）.There was no significant difference in complication between the two groups（P0.05）.Conclusion Another choice of vascular access for a hemodialysis patient is placement of long-term Dacro-cuffed dural lumen catheter through external Jugular.Especially for Jugular vein puncture failure,Jugular vein catheter infection requires replacement and external jugular vein were exposed coarse.
