血液透析体外循环动脉端与静脉端输血对透析器凝血的影响比较

摘　　要：目的探讨维持性血液透析并重度贫血患者在血液透析体外循环动脉端和静脉端途径输血对透析器凝血的影响。方法对20例维持性血液透析并重度贫血患者采用自身对照方法,分别是:利用加压输液袋在透析器后端的静脉管路静脉壶输血,直接在透析器前端的动脉管路输液口输血,2种方法各进行60例次,观察比较2种方法透析器外观凝血及透析器血室容量下降率。结果与常规在动脉端输血比较,利用加压输液袋在体外循环静脉端输血,透析器外观凝血和透析器血室容量下降率均减少,差异均有统计学意义(P<0.05)。结论对于维持性血液透析并重度贫血患者,利用加压输液袋在体外循环静脉端输血,降低了透析器凝血的风险,减少了患者的痛苦,更好地保证了透析质量。Objective To explore the influence of blood transfusion of extracorporeal circulation in hemodialysis on dialyzer coagulation.Methods 20 maintenance hemodialysis patients with serious anemia were divided two groups with self-control method： the routine dialysis with artery ends blood transfusion in front of dialyzer（control group） and vein end blood transfusion by compression infusion bag at the end of dialyzer（observation group）.Methods in two groups were carried for 60 times.Surface coagulation of dialyzer and decreasing rate of dialyzer capacity were observed in two groups.Results Compared with the common artery end blood transfusion,the incidence of blood coagulation and decreasing rate of dialyzer capacity in the vein end blood transfusion by compression infusion bag obviously reduced,both with statistical significances（P〈0.05）.Conclusion The vein ends blood transfusion by compression infusion bag in hemodialysis external circulation can reduce the risk of dialyzer coagulation and preferably guarantee the quality of the hemodialysis.
