集束干预策略预防鼻饲患者呼吸机相关性肺炎的效果观察

摘　　要：目的探讨集束干预策略对预防鼻饲患者呼吸机相关性肺炎的效果。方法对2009年1—12月收住呼吸科重症监护病房的408例机械通气鼻饲患者进行集束护理干预策略:鼻饲时采取左侧卧位、抬高床头30°～45°;鼻饲前监测气囊压力;监测胃内残余量;加强口咽部护理。选择2008年1—12月同科室行机械通气采取常规鼻饲护理方法的348例患者作为对照组。比较两组患者呼吸机相关性肺炎的发生情况。结果集束组呼吸机相关性肺炎发生率较对照组明显下降(χ2=25.748,P<0.01)。结论集束干预策略能减少鼻饲患者呼吸机相关性肺炎的发生率,值得临床推广应用。Objective To explore the effect of bundle of care about nasogastric tube feeding on preventing ventilator-associated pneumonia（VAP）.Methods 408 patients undergoing mechanical ventilation from January to December 2009 in RICU were selected as experimental group and other 348 patients with mechanical ventilation cases from January to December 2009 as control group.In experimental group,bundle of care about nasogastric tube feeding was applied： monitoring the air pressure of air sac before nasogastric tube feed,placing patients in left lateral recumbent position with bed head angled from 30° to 45°,monitoring remaining food in gastric,reinforcing oral care.And in control group,traditional nasogastric tube feeding was applied.The incidence of VAP in two groups was observed.Results Incidence of VAP in experimental group was lower than that in control group（χ^2=25.748,P 〈0.01）.Conclusion Bundle of care about nasogastric tube feeding can reduce the incidence of VAP.
