老年慢性心功能衰竭患者院内外健康教育的研究

摘　　要：目的探讨全程健康教育对老年慢性心功能衰竭患者的影响,为老年慢性心功能衰竭患者出院后的护理模式提供参考依据。方法纳入108例慢性心功能衰竭住院患者作为研究对象,随机分为对照组和观察组各54例,对照组按照传统方法进行规范的健康教育,观察组实施全程健康教育1年。比较两组6 min步行试验结果、因心功能衰竭再住院率和死亡率、再住院平均住院天数和医疗费用、各项生活质量评分。结果观察组因心功能衰竭再次住院例数、平均住院天数、死亡例数均明显低于对照组,同时降低了医疗费用(P<0.05);观察组患者6 min步行距离、功能性日常生活能力、老年人生活质量的评分均高于干预前及对照组(P<0.05)。结论全程健康教育干预可改善患者的心脏功能,促进康复,降低再住院率和死亡率,减少住院天数和医疗费用,提高患者的生活质量,提示老年心功能衰竭患者的健康教育应延续到家庭护理中。Objective To explore the effect of comprehensive health education on elderly patients with chronic heart failure（CHF） in and out of hospital and to provide a nursing model after discharge.Methods 108 patients with CHF were dividend into experimental group and control group randomly.In control group,traditional health education was introduced.While in experimental group,comprehensive healthy education lasted for one year.Comparisons were made about the results of 6-minitue-walk test,readmission rate and mortality,the mean time of hospitalization,and medical costs,the quality of life score as well.Results Patients in experimental group were with less patients hospitalizing for heart failure once again,less mean hospitalization time,less mortality compared with those in control group.Meanwhile,health care cost reduced in experimental group as well（P〈0.05）.Patients performed better in experimental group in the 6-minitue-walk test,functional activities of daily life,quality of life scores（P〈 0.05）.Conclusion Comprehensive health education intervention can improve the patients’ cardiac function,promote rehabilitation and reduce readmission rate,mortality,the mean hospitalization time and medical costs then finally improve life quality of patients.It is suggested that health education for elderly patients with heart failure should be introduced to homecare.
