109例吻合器痔上黏膜环切术后3年疗效分析

摘　　要：目的探讨吻合器痔上黏膜环切术(PPH术)后3年的疗效及并发症。方法总结109例PPH术患者的临床资料及随访情况。结果出现远期并发症总共58例(53.21%),平均满意度为82.70±15.85分,其中不同程度的出血28例(25.69%),肛门疼痛12例(11.01%),坠胀15例(13.76%),排便困难25例(22.94%),吻合口狭窄3例(2.75%),残留皮赘41例(37.61%),复发4例(3.67%),再次手术17例(15.60%)。结论 PPH术有一定的远期并发症,可通过严格掌握适应症,规范手术操作、提高操作技巧,积极处理并发症来预防。Objective To evaluate postoperative efficacy and late postoperative complications of 109 cases with procedure for prolapse and hemorrhoids after three years. Methods The clinical data of 109 cases were treated with PPH and the information of follow-up to them were analyzed retrospectively. Results The average satisfaction was 82.70±15.85,and there were 58 cases（53.21%）with late postoperative complications,including different extent bleeding （28 cases）（25.69%）,pain （12 cases）（11.01%）,local discomfort （15 cases）（13.76%）,difficult defecation （25 cases）（22.94%）,stenosis of anastomotic stoma （3 cases）（2.75%）,cutaneous tag （41 cases）（37.61%）,recurrence （4 cases）（3.67%）,reoperation （17 cases）（15.60%）. Conclusion PPH has certain late postoperative complications,and could be precautioned by controlling indication strictly,standard operative procedure,improving operative skill,and treat complications positively.
