肝纤维化与血浆肿瘤坏死因子-α、白细胞介素-6和白细胞介素-10水平的关系

摘　　要：目的探讨肝纤维化患者血浆中促炎细胞因子与抗炎细胞因子水平的变化及其与肝纤维化指标的关系。方法检测137例肝纤维化患者血浆肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)及透明质酸(HA)、层黏蛋白(LN)、Ⅲ型前胶原N端肽(PⅢNP)、Ⅳ型胶原-C(Ⅳ-C)水平,并进行Child-Pugh分级和分析。结果肝纤维化患者血浆TNF-α、IL-6、HA、LN、PⅢNP、Ⅳ-C水平较对照组均明显升高(P<0.05或P<0.01),IL-10水平较对照组均明显降低(P<0.05),并随Child-Pugh评分增加而递增或递减(P<0.05或P<0.01)。TNF-α、IL-6与HA、LN、PⅢNP、Ⅳ-C水平间均呈显著正相关(P<0.05或P<0.01),IL-10与HA、LN、PⅢNP、Ⅳ-C水平间均呈显著负相关(P<0.05)。结论 TNF-α、IL-6、IL-10与肝纤维化密切相关,在肝纤维化的早期诊断中具有一定的临床价值;抑制TNF-α、IL-6的活性,加强IL-10的作用,可能是治疗肝纤维化的有效途径和手段。Objective To study the expressiun of pro - and anti - inflammatory cytokines in patients with hepatic fibrosis, and investi-gate the relationship of pro - and anti - inflamatory eytokines with hepatic fibrosis indexes. Methods The plasma levels of tumorr necrosis factor （TNF） -α interleukin-6（IL-6）, hyaluronic acid（HA）, laminin（LN）, interleukin- 10（IL- 10）. collagen type m N -terminal wptide（Pm NP） ,and collagen IV （IV -C） were determined in 137 patients with hepatic fibrosis, and Child -Pugh score were recorded and compared statistically. Results In 137 patients with hepatic fibrosis, the plasma levels ofTNF - α,IL-6, HA, LN, emNP, and IV -C were. significantly higher than those in norml control subjects （P 〈0. 05 or P 〈0.01 ）, whereas the plasma levels of IL - 10 were significantly lower than those in norreal control subjects （P 〈0.05）, There was significant positive correlation of expression levels of TNF -α, IL-6 and HA, LN, PⅢNP,and Ⅳ -C（P 〈0.05 or P 〈0. 01 ）, There was significant negative correlation of expression levels of IL- 10 and HA, LN, PⅢNP, and Ⅳ- C（P 〈0. 05）. Conclusions The plasma levels of TNF-α,IL -6, and 1L 10 have a close relationship with hepatic fibrosis. They may be. valuable indicators for the diagnosis of hepatic fibrosis in early stage, and may be an efficient approach for treatment of hepatic fibrosis.
