双管型喉罩通气对老年全麻患者呼吸功能的影响

摘　　要：目的:观察应用双管型喉罩通气对老年全麻患者呼吸功能的影响。方法:选择拟行全麻的老年患者60例,随机分为喉罩组(L组)和气管插管组(E组),每组30例,观察气管插管或放置喉罩后1min(T1)、5min(T2)、10min(T3),拔除气管导管或喉罩时(T4)的呼气末二氧化碳(EtCO2)、气道峰压(Peak)及血氧饱和度(sPO2),并记录术毕拔除气管导管和喉罩时间,及有无呛咳、恶心、高血压、躁动等不良反应。结果:L组在放置喉罩后与E组气管插管后1min(T1)、5min(T2)、10min(T3)时点EtCO2、Peak、sPO2比较差异无统计学意义(P>0.05),L组在拔除喉罩时(T4)的Peak低于E组,L组拔除喉罩时呛咳、高血压的发生率明显低于E组(P<0.05),L组拔除喉罩时间短于E组拔除气管导管时间。结论:老年全麻患者应用双管型喉罩通气,减少了气管插管引起的呼吸系统并发症,改善了患者术后的呼吸功能,提高了老年患者围术期安全性。Objective：To observe the effect on respiratory function of elderly patients after the use of Proseal laryngeal mask airway for mechanical ventilation during general anesthesia.Methods：In this randomized,double-blind study,60 old-age patients were allocated to one of two groups（n=30）,laryngeal mask group（Group L）and tracheal catheter group（Group E）.End-tidal CO2（EtCO2）and airway peak pressure（Peak） and pulse oximetry oxygen saturation（sPO2）were monitored and recorded at 1min（T1）,5min（T2） and 10min（T3）after putting the masks or catheters in right place and before pulling out those tubes（T4）.Any incidents of bucking,nausea,hypertension and dysphoria would be recorded during the pulling process,so did the using time from end-operation to the laryngeal mask or tracheal catheter pulled out.Results：There were no significant difference in EtCO2,Peak and sPO2 1min after laryngeal mask or tracheal catheter placed rightly（P0.05）.The peak pressures at T4 were lower in Group L than in Group E,as well as the rates of bucking and hypertension（P0.05）,and we also found that the using time was smaller in Group L than in Group E.Conclusion：Application of the laryngeal mask in elderly group can improve respiratory function by reducing the incidents of complications of trachea cannula,and increase the elderly men security during general anesthesia.
