玉屏风颗粒对儿童肾病综合征免疫功能影响的研究

摘　　要：目的探讨玉屏风颗粒防治儿童原发性肾病综合征(PNS)免疫功能低下的疗效。方法采用前瞻性随机的方法,通过查随机数字表将病例分为治疗组和对照组,两组治疗均采用泼尼松中长程疗法,治疗组加用玉屏风颗粒,5 g/次,3次/d,每服药20 d间隔7 d,疗程为12个月,观察玉屏风颗粒对患儿感染和复发、血清IgG、IgA水平和T细胞亚群及血浆白蛋白(Alb)、24h尿蛋白的影响。结果治疗组继发性感染发生率和复发率均低于对照组,差异有统计学意义(P均(0.05);治疗组血清IgG、IgA水平高于对照组,差异有统计学意义(P均(0.01);治疗组总T细胞(CD3+)与治疗前比较,差异有统计学意义(P(0.01),与对照组治疗后比较,差异有统计学意义(P(0.05);CD4+细胞比率与治疗前比较,差异有统计学意义(P(0.01),与对照组治疗后比较,差异有统计学意义(P(0.01);CD8+细胞比率与治疗前比较,差异有统计学意义(p(0.01),与对照组治疗后比较,差异有统计学意义(P(0.01);CD4+/CD8+比值与治疗前比较,差异有统计学意义(P(0.01),与对照组治疗后比较,差异有统计学意义(P(0.01);血浆白蛋白高于对照组,24h尿蛋白低于对照组,差异均有统计学意义(P均(0.01)。结论玉屏风颗粒可调节PNS患儿免疫状态,对防止PNS复发和治疗困难有积极作用。Objective To explore the therapeutic effect of Yupingfeng Powder in secondary immunodeficiency caused by primary nephrotic syndrome in children.MethodsAll patients were divided into treatment group and control group by means of prospective random number table.The scheme of mid-phase prednisone was used in treatment group and control group.The treatment group was given Yupingfeng Powder after prednisone,5g three times daily,for each medication 20d with a seven day interval,treatment for 12 months.The influence of Yupingfeng Powder on the infection and recurrence,serum IgG,IgA level and T cell subsets and plasma albumin（Alb）,24h urinary protein of the children were observed.ResultsThe incidence of secondary infection and recurrence rates in treatment group were lower than those in control group（P﹤0.05）.The levels of IgG,IgA in treatment group were higher than those in control group（P﹤0.05）.There had significant difference（P0.01） for CD3＋ cells,CD4 ＋ cell ratio,CD8＋ cell ratio,CD4 ＋ / CD8 ＋ in treatment group before and after treatment.There was a significant difference between the two group compared by CD3＋ cells after treatment（P0.05）.The CD4 ＋ cell ratio,CD8 ＋ cell ratio,CD4 ＋ / CD8 ＋ were significantly different in the treatment group as compared to the control group before and after treatment（P0.01）.The plasma albumin（Alb） in treatment group was significantly higher than that in control group（P0.01）,and the 24h urinary protein in treatment group was lower than that in control group（P0.01）.ConclusionYupingfeng Powder has function to adjust immunological function,and play an active role to prevent recurrence of PNS and to patients with difficult to treat.
