摘 要:目的观察自发性高血压大鼠(SHR)血压增高的初期、增高期和稳定期左室肥厚和心肌纤维化参数的改变,探讨高血压左室肥厚和心肌纤维化的动态演变规律。方法应用生化测定、病理检查结合计算机分析等方法,检测SHR及其对照WKY在6周、14周和24周的收缩压、左室重量及左室重量指数、心肌细胞面积及横径、心肌胶原容积分数(CVF)、血管周围胶原面积(PVCA)及心肌组织内羟脯氨酸浓度。结果SHR左室重量及左室重量指数、心肌细胞面积及横径在高血压初期(6周)无明显改变,14周时有显著增加(S14vsW14,S14vsS6,P<0.05),24周时增加更加显著(S24vsW24,P<0.01;S24vsS14,P<0.05);CVF在6周和14周均无明显变化,24周时显著增加(S24vsW24,S24vsS14,P<0.05);PVCA在高血压初期即已有显著改变(S6vsW6,P<0.05),14周时显著增加(S14vsW14,S14vsS6,P<0.05),24周时仍有增加,但未达到显著性标准(S24vsW24,P<0.05;S24vsS14,P>0.05);心肌组织内羟脯氨酸浓度(HC)在14周时开始有显著性增加(S14v?Aim \ By means of observing the changes of the parameters representing the left ventricular hypertrophy(LVH) and myocardial fibrosis(MF) of spontaneously hypertensive rats(SHRs) in the early stage, increasing stage and stable stage of hypertension formation, to investigate the dynamic changing tendency of LVH and MF.\ Methods \ Using the methods of biochemical assay and pathological examination combined with computed processing, the systolic blood pressure (SBP), left ventricular weight(LV) and left ventricular index(LVI), cardiomyocytes size and diameters, collagen volume fraction(CVF), perivascular circumferential area(PVCA) and the hydroproline concentration(HC) of the heart in the 6, 14 and 24 weeks old of SHRs and the control WKY were studied.\ Results \ The LV, LVI, cardiomyocyte size and diameters had no great alterations in the 6th week, but increased markedly in the 14th week(S14 vs W14, S14 vs S6, P<005) and more significantly in the 24th week of SHRs (S24 vs W24, P< 001;S24 vs S14,P< 005) CVF raised only in the 24th week (S24 vs W24, S24 vs S14,P<005) while the increases in PVCA at the early stage(S6 vs W6, P<005) and increased gradually (S14 vs W14, S14 vs S6, S24 vs W24, P<005;S24 vs S14, P>005). The HC in the heart of SHRs raised significantly in the 14th week (S14 vs W14, P<005) and sustained to 24th week (S24 vs W24, S24 vs S14, P<005).\ Conclusion \ With the development of hypertension, cardiomyocyte hypertrophy, myocardial fibrosis (MF), as well as interstitial and perivascular fibrosis developed in a various manner, indicating different underlying mechanisms.
