DNA免疫吸附治疗抗双链DNA抗体阳性系统性红斑狼疮患者的临床疗效观察

摘　　要：目的探讨应用DNA280免疫吸附治疗抗双链DNA(ds-DNA)抗体阳性系统性红斑狼疮(SLE)患者的临床疗效和安全性。方法选择2009年1月—2010年2月我院住院的12例抗ds-DNA抗体阳性SLE患者,进行DNA免疫吸附(IA)治疗,对比治疗前后的抗核抗体(ANA)与抗ds-DNA抗体滴度、常规生化检查、补体水平及系统性红斑狼疮病情活动性指数(SLEDAI)评分,观察疗效和不良反应。结果 DNA免疫吸附治疗后ANA、抗ds-DNA滴度、IgG及IgA水平较治疗前显著下降,差异均有统计学意义(P<0.05)。DNA免疫吸附治疗后白细胞计数较吸附前显著升高,差异有统计学意义(P<0.05)。血小板计数、血肌酐、血钾、IgM、C3、C4治疗前后差异均无统计学意义(P>0.05)。DNA免疫吸附治疗后患者SLEDAI评分较治疗前显著下降,差异有统计学意义(P<0.05)。2例患者治疗后出现血小板计数下降,其余患者无不良反应。结论 DNA免疫吸附治疗抗ds-DNA抗体阳性SLE患者有效,可降低体内ANA、抗ds-DNA抗体滴度,使SLE病情活动度下降,且具有较高的安全性。Objective To investigate the clinical efficacy and safety of the treatment of DNA-immunoadsorption （DNA-IA） in systemic lupus erythematosus （SLE） patients with the anti-double-stranded DNA （ds-DNA） positive.Methods Twelve anti-ds-DNA-positive SLE inpatients were selected and treated with DNA-IA in our hospital from January 2009 to February 2010. The changes of antinuclear antibody （ANA） and ds-DNA titers,routine biochemical examination,complement level,and SLEDAI score before and after the treatment were observed. And the adverse reactions and complications were also examined.Results After treatment with DNA-IA,the titers of ANA and ds-DNA,the levels of IgG and IgA were significantly decreased（P0.05）. The white blood cells count was increased significantly （P0.05）. The changes of platelet count,serum creatine,kalium,IgM,C3,and C4 showed no statistically significance（P0.05）. SLEDAI score was significantly decreased （P0.05）. The decreased platelet count was found in two patients,and no adverse reaction was found in the rest patients.Conclusion The treatment of DNA-IA in the anti-ds-DNA-positive SLE patients can decrease the titers of ANA and ds-DNA antibody,it can decrease the disease activity of SLE and is of higher safety.
