摘 要:目的探讨外阴阴道念珠菌病(VVC)白念珠菌基因多样性与抗真菌药物敏感性的关系。方法采用最近开展的CAI微卫星法聚合酶链反应-单链构象多态性(PCR-SSCP)技术鉴定VVC患者白念珠菌的基因型,采用E-test法对检出的念珠菌进行体外药敏试验。结果从VVC患者分离出89株白念珠菌,鉴定出26种基因型。白念珠菌对两性霉素B、氟胞嘧啶、酮康唑和氟康唑敏感。主要基因型(A、B、C、D)占69.7%(62/89),白念珠菌B基因型菌株及非B基因型菌株对伊曲康唑(ITR)的耐药率分别为66.7%(14/21)和4.4%(3/68),差异有统计学意义(P<0.05)。结论 PCP-SSCP技术有利于研究白念珠菌对抗真菌药物的耐药机制。白念珠菌B基因型菌株对ITR有更高的耐药性。Objective To investigate the relationship between genetic diversity of Candida albicans strains and antifungal susceptibility in patients with vulvovaginal candidiasis (VVC).Methods The genotypes of Candida albicans in patients with VVC were studied by using a recently developed polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) of CAI microsatellite method, and antifungal susceptibility was tested by using E-test methods.Results Twenty-six genotypes were identified from 89 strains of Candida albicans isolated from patients with VVC. The isolated Candida albicans were susceptible to amphotericin B, flucytosine, ketoconazole and fluconazole. The dominant genotypes (A, B, C, D) accounted for 69.7% (62/89) of Candida albicans. The resistant rates of Candida albicans genotype B and non-genotype B strains to itraconazole (ITR) were 66.7% (14/21) and 4.4% (3/68), respectively.Conclusion The Candida albicans genotype B form patients with VVC was more resistant to ITR.
