摘 要:目的:探讨两种加温输液液体加温效果及对患者术中体温的影响。方法:60例行乳腺癌根治术患者随机分为两组,每组30例,恒温水箱组(HW组):患者手术全程输入经恒温水箱加温至40℃的液体;专用电子输血输液加温仪组(JW组):患者手术全程输入经专用电子输血输液加温仪加温的40℃液体。每例患者输入的液体建立两个测温点:tⅠ(输液瓶中液体温度)及tⅡ(输液管患者端液体温度)。监测患者麻醉前、全麻插管后15min、30min、60min、90min、120min、150min及术毕体温(BT)、tⅠ及tⅡ变化。结果:两组各时间点tⅠ比较,JW组明显低于HW组(P<0.01);全麻插管后各时间点tⅡ比较,JW组均明显高于HW组(P<0.01);与各时间点tⅠ比较,HW组患者在全麻插管后各时间点tⅡ均明显降低(P<0.01),而JW组患者各时间点tⅡ均明显升高(P<0.01);两种方式加温输液中,随时间延长,HW组tⅠ、tⅡ逐渐下降,全麻插管后60mintⅡ已明显低于tⅠ(P<0.05或P<0.01),而JW组各时间点tⅡ均明显高于同时点tⅠ,亦高于同时间点HW组tⅡ(P<0.01);与全麻插管前患者体温比较,HW组患者体温在全麻插管后15min、30min、60min无明显改变,90min后明显降低(P<0.05或P<0.01),而JW组患者手术全程体温无明显变化。结论:与静脉输入恒温水箱加温液体相比,输入经专用输血输液加温仪加温液体能提供更确切的液体温度,更能保障术中患者的体温趋于正常水平。Objective:To explore the effect of two menthods of heated transfusion on the liquid heating effect and the body temperature of patients during operation. Methods: Radical mastectomy for 60 patients with breast cancer were randomly divided into two groups equally: group HW (constant-temperature water-bath group) and group JW (special electronic heated instrument of blood and fluid transfusion group) , each group was 30 case. During the operation, group HW was given 40℃ transfusion heated by constant-temperature water-bath, group HW was given 40℃ transfusion heated by special electronic heated instrument of blood and fluid transfusion. Two points of liquid temperature in each case were recorded: tⅠ(the liquid temperature of infusion bottle)and tⅡ(the liquid temperature of patient end infusion tube). The changes of body temperature,tⅠ and tⅡ were measured before anesthesia and 15,30,60,90,120,150 min after anesthesia intubation and at the end of operation.Results: The liquid temperature of infusion bottle (tⅠ) of the group JW was significant lower than the group HW at every time points(P〈0.01); the liquid temperature of patient end infusion tube (tⅡ) of the group JW was significant higher than the group HW at every time points after anesthesia intubation(P〈0.01); Compared with the every time point of tⅠ, the every time point of tⅡ was low significant in group HW patient(P〈0.01)and was high significant in group HW patient after anesthesia intubation(P〈0.01). In the two heated transfusion mode, with time passing by, tⅠ,tⅡ in group HW decreased gradually, tⅡ was lower significant than tⅠ after anesthesia intubation(P〈0.05 or P〈0.01), at every time point of group JW, tⅡ was higher significant than tⅠ, and was higher significant than group HW tⅡ at the same time point.
