疏血通注射液联合环磷腺苷葡胺注射液治疗围生期心肌病的临床观察

摘　　要：目的探讨疏血通注射液联合环磷腺苷葡胺注射液治疗围生期心肌病(PPCM)的临床疗效。方法将60例PPCM伴有心功能不全患者随机分为对照组30例和治疗组30例;2组患者均常规给予利尿剂、ACEI(或ARB)、β受体阻断剂等常规治疗,心脏超声提示附壁血栓时使用抗凝药物,而治疗组在常规治疗的基础上给予疏血通注射液及环磷腺苷葡胺注射液。2组疗程均为2周。治疗前后行NYHA分级、心脏彩色多普勒超声、血液流变学等相关指标测定。结果治疗组总有效率为93.3%高于对照组的80.0%,差异有统计学意义(P<0.05)。2组治疗后的平均LVEF较治疗前有明显好转(P<0.05或P<0.01),且2组间比较差异有统计学意义(P<0.05)。治疗组治疗后全血黏度、血浆黏度、红细胞聚集指数等指标均改善,与治疗前比较差异有统计学意义(P<0.05);而对照组治疗后上述指标虽有一定的改善,但差异无统计学意义(P>0.05);治疗后2组比较差异有统计学意义(P<0.05)。结论在常规抗心力衰竭治疗的基础上联合疏血通注射液+环磷腺苷葡胺注射液治疗,可明显改善围生期心肌病患者的心功能,值得在临床上广泛应用。Objective To discuss the clinical effect of shuxuetong injection combined with meglumine adenosine cyclophosphate injection in perinatal period myocardiosis（PPCM）.Methods 60 patients with PPCM associated with heart failure were randomly divided into control group（30 cases）and the treatment group（30 cases）;Two group were given conventional diuretic,ACEI（or ARB）,β receptor blocking agents,cardiac ultrasound tips mural thrombus in the use of anticoagulant drugs in the treatment group on the basis of conventional therapy for shuxuetong injection and meglumine adenosine cyclophosphate injection.2 group course of treatment for 2 weeks.Line NYHA classification before and after treatment,echocardiography and blood rheology and related Indexes.Results The total effective rate of treatment group（93.3%）was higher than that in control group（80.0%）,the difference was statistically significant（P0.05）.After treatment,the average LVEF of two groups significantly improved（P0.05 or P0.01）;After treatment the average LVEF of two groups was statistically significant（P0.05）.The whole blood viscosity,plasma viscosity,erythrocyte aggregation index and other indicators of treatment group were to a certain improvement,compared with that before treatment,the defference was significantly（P0.05）;The indicators of control group had some improvement,but the difference was not statistically significant（P0.05）;After treatment,the difference between 2 groups was statistically significant（P0.05）.Conclusion In the conventional therapy based on the joint to shuxuetong injection＋meglumine adenosine cyclophosphate injection,can significantly improve perinatal cardiac function in patients with myocardiosis,widely used in clinical practice.
