彩色多普勒超声诊断感染致婴儿肝炎综合征临床价值研究

摘　　要：目的探讨无创性超声对感染致婴儿肝炎综合征(婴肝征)疾病诊断及治疗效果、疾病转归的临床应用价值。方法选取感染致婴肝征患儿,分别于急性期、恢复期及临床治愈期探测肝静脉、肝动脉及门静脉的内径(DPV)、门静脉最大血流速(PVVmax;肝动脉收缩期最大血流速(PSV)、舒张期最大血流速(EDV)及阻力指数(RI)、肝静脉频谱波形(HVSW)特点。结果 (1)病例组比对照组肝脏轻度肿大、DPV增宽、PVVmax增快、PHAPSV增快,PHA RI增高、DHV减小。2组比较差异有统计学意义(P<0.01)。(2)PHAEDV、DPHA 2组比较差异无统计学意义(P>0.05)。(3)病例组HVS以HV1型为主,对照组以HV0型为主。(4)相关分析表明,PHAPSV和PHARI及HVSW与感染致婴肝征病情程度呈较好正相关(r=0.87和0.78,r=0.55,P<0.01)。结论肝脏超声血流动力学及频谱波形较客观地反映了感染致婴肝征的病情,为临床提供更多的信息,具有临床应用价值,有望成为感染致婴肝征诊断、治疗效果评价及预后判断的一种无创、有效的方法。Objective To explore the clinical value of color Doppler ultrasonography in the diagnosis of infant hepatitis syndrome caused by infection. Methods Totally 33 cases of clinical confirmed infant hepatitis symptoms caused by infection were selected, and in the acute stage, convalescence and Clinical cure periods, the internal diameters of hepatic vein and hepatic artery and portal vein, the largest blood velocity in the portal vein and hepatic artery maximum blood velocity, diastolic maximum blood flow velocity and RI, hepatic venous wave characteristics were detected. In addition, 39 cases of health infants with the similar months were selected as the controls. Results （ 1 ） Compared with the healthy control group, the diameters of portal main vessel of the case group were wider （ P （ 0.01 ）, and the maximum flow increased （P〈0.01）. （2） For PHAEDV and DPHA, there were no differences between the two groups （P〉0.01）; （3） For HVS, in the case group, HV1 was the main type, and in the control group, HV0 was the main type. （4） The correlation analysis showed that PHAPSV, PHARI and HVSW were positively correlated with the degrees of sickness of infant hepatitis syndrome caused by infection （ P 〈0.01 ）. Conclusions Color Doppler ultrasonography has high clinical values for infant hepatitis syndrome caused by infection.
