卵黄抗体用于大山区血吸虫病诊断的可行性

摘　　要：目的评价卵黄抗体(anti-SEA chicken egg yolk antibody,IgY)用于血吸虫病诊断在大山区型血吸虫病流行现场的应用效果。方法对大山区血吸虫病流行区的506位居民进行粪检,同时用IgY(以IgY作为捕捉抗体,以酶标NP28-5B为检测抗体)和血吸虫抗体ELISA检测试剂盒(SEA-ELISA)分别对其血清进行检测,评价anti-SEA IgY的敏感性,分析其用于大山区血吸虫病诊断的可行性;同时检测非流行区的200份血清确定其特异性。结果 IgY和SEA-ELISA的粪检阳性符合率分别为92.68%(152/164)和89.02%(146/164);两者的特异性分别为99%(198/200)和95%(190/200),差异无显著性(P均>0.05),但在粪孵阴性人群血清样本中,IgY和SEA-ELISA的检出率分别为31.00%(106/342)和83.63%(286/342),差异有显著性(P<0.05)。结论 anti-SEA IgY可用于大山区血吸虫病诊断。Objective To evaluate the potency of IgY for the diagnosis of schistosomiasis in the mountainous endemic spots.Methods A total of 506 inhabitants in the mountainous schistosomiasis endemic regions were examined by using the stool hatching test for parasitological stool examination;the sera samples from 506 villagers were tested with IgY（using IgY as the capture antibody and NP28-5B conjugated with HRP as detection antibody） and SEA-ELISA for immune examination respectively,and the sensitivity of IgY was estimated.Analyze the feasibility of IgY used for the diagnosis of schistosomiasis in mountainous areas.Their specificity was determined by testing 200 sera from nonepidemic scenes.Results The positive coincidence of stool hatching test with IgY and SEA-ELISA were 92.68%（152/164） and 89.02%（146/164）respectively,and their specificity as 99.00%（198/200） and 95.00%（190/200）,with no significant difference between them（P〉0.05）.However,in the negative sera from stool hatching test,the detection rate was 31.00%（106/342） by IgY,and 83.63%（286/342）by SEA-ELISA,the difference was significant（P〈0.05）.Conclusion IgY could be used in the mountainous area for the diagnosis of schistosomiasis.
