摘  要:目的探讨儿童幽门螺杆菌(Hp)相关性胃十二指肠疾病中胃黏膜基质金属蛋白酶-9(MMP-9)的表达情况。方法对具有上消化道不适主诉症状的儿童(包括66例慢性胃炎及30例十二指肠溃疡患儿)经上消化内镜检查获取胃黏膜活检标本。根据Sydney胃炎标准对HE染色的活检组织进行胃窦黏膜慢性炎症程度评价。血清Hp-IgG抗体检测、聚合酶链反应(PCR)、尿素酶试验及组织学检查作为确定Hp感染状态的依据。所有病例采用逆转录聚合酶链反应(RT-PCR)方法及免疫组化法检测胃黏膜MMP-9的表达。结果胃黏膜MMP-9 mRNA阳性表达率及表达水平在不同的Hp感染状态中无明显差异,其表达水平与胃窦黏膜慢性炎症程度无关。免疫组化染色显示MMP-9主要在炎症细胞中表达。结论 Hp感染对儿童胃十二指肠疾病的胃黏膜MMP-9的表达无影响。Objective To determine the expression of matrix metalloprotease-9(MMP-9) response in children with and without H.pylori infection.Methods Gastric biopsies were obtained from children with dyspeptic symptoms.MMP-9 expression was assessed in 66 patients with chronic gastritis and 30 patients with duodenal ulcer by reverse transcription polymerase chain reaction biopsy-based tests and by immuno-histochemistry using specific antibodies.Biopsy specimens were stained with hematoxilin and eosin(HE),and the degree of antrum chronic inflammation was graded according to the Sydney system.Serology,PCR,urease test and histology were used to assess H.pylori status.Results The positivity and the level of gastric mucosal MMP-9 mRNA expression were not different between H.pylori-infected and uninfected patients.MMP-9 mRNA was not correlated with the degree of antrum chronic inflammation.Immunohistochemical expression of MMP-9 was predominantly observed at inflammatory cells.Conclusion H.pylori has no effect on MMP-9 expression in gastric mucosa in children with upper gastrointestinal disease.
