老年初治肺结核合并糖尿病65例临床分析

摘　　要：目的探讨老年糖尿病与肺结核并发时的临床特点。方法对我科收治的65例60岁以上的老年糖尿病合并肺结核患者的临床资料及治疗情况进行分析。结果 65例老年糖尿病合并肺结核患者中,胸片及肺CT以干酪渗出为主要表现者48例(73.85%),病变累及3个及以上肺野30例(46.15%),合并空洞26例(40%)。经治疗后,所有患者症状都有不同程度的缓解,其中40例(61.64%)症状明显好转,痰菌阴转36例(85.71%),病灶吸收好转58例(89.23%),血糖控制较好(空腹血糖<8.0mmol/L)54例(83.08%)。结论老年人糖尿病并发肺结核发病率高,肺结核的治疗与糖尿病的控制情况密切相关,需要两病同治,早发现、规则治疗,对防止并发症非常重要。Objective To explore the clinical characteristics of pulmonary tuberculosis combined with diabetics in the aged.Methods Clinical data of 65 cases from institute of tuberculosis in the 309th Hospital of PLA were collected and analyzed.Results Of 65 cases,48 cases showed effusion lesions on their chest X-ray and lung CT,and 26 cases had cavities.After being treated,the symptoms of 40 cases were improved obviously,sputum negative conversion was found in 36 cases（85.71%）,focus of infection was ameliorated in 58 cases（89.23%）,the blood glucose was found lower than 8.0 mmol/L in 54 cases（83.08%）.Conclusion Incidence of pulmonary tuberculosis in elderly diabetics is higher.The prognosis is closely related to the control of diabetes.The simultaneous treatment of two diseases,early and regular chemotherapy are very important for the good prognosis.
