应用MIPO技术小切口治疗老年肱骨近端骨折

摘　　要：目的讨论应用微创接骨板固定术(minimally invasive plate osteosynthesis,MIPO)治疗老年肱骨近端骨折的临床意义及手术方法。方法运用小切口MIPO技术治疗2007年8月至2009年8月34例Neer分型二～四部分老年肱骨近端骨折,其中男12例,女22例;年龄61～82岁,平均71.4岁。对其进行6～24个月随访观察。结果运用MIPO技术治疗老年肱骨近端骨折,手术时间短,术中出血量少,没有发生神经血管损伤、骨折不愈合和内固定松动、断裂等并发症,术后患者肩关节功能恢复优21例,满意10例,不满意3例,优良率91%。结论 MIPO在治疗老年肱骨近端骨折时以微创来提供尽可能少受干扰的骨折愈合环境,具有创伤小、固定确实、感染率低、骨折愈合率高等优点,减少了并发症的发生,符合生物学固定的原理。Objective To discuss the application of minimally invasive plate fixation（MIPO） in the treatment of proximal humeral fractures in elderly clinical significance and surgical methods.Methods From August 2007 to August 2009,34 cases of elderly proximal humeral fractures whichwas Neer classification of two～four-part were conducted with MIPO treatment of minor incision.Results MIPO treatment of the elderly proximal humeral fractures have many advantages：less operative time,less blood loss,no neurovascular injury occurred,no fracture unhealing and fixation loosening,no fracture and other complications.Postoperative functional recovery in patients was excellent in 21 cases of shoulder,satisfactory in 10 cases,unsatisfactory in 3 cases,good rate was 91%.Conclusion MIPO treatment of proximal humeral fractures in the elderly to provide a minimally invasive disturbed as little as possible fracture healing environment,with less trauma and a small,fixed Indeed,the infection rate is low,high rate of fracture healing benefits,reducing the incidence of complications consistent with Biological fixation principle.
