摘　　要：观察３０例慢阻肺合并Ⅱ型呼吸衰竭患者，应用ＢｉＰＡＰ双水平压力支持通气治疗的效果，其中１５例用单向阀，另１５例不用单向阀进行机械通气治疗，并以同期同样标准１５例患者作为对照组，三组治疗前后均做动脉血气分析，结果显示：单向阀组经通气治疗后有明显改善通气，提高ＰａＯ２而降低ＰａＣＯ２作用，而未使用单向阀组仅提高ＰａＯ２而ＰａＣＯ２无明显改善。鉴于它的无创性特点，有益于临床推广使用。文中对ＢｉＰＡＰ呼吸机的优缺点及注意事项进行了讨论。Observing 30 Cases with chronic obstructive disease of lung(typeⅡ).Applying BiPAP,15 cases with one-way valve,the other 15 with no one-way valve mechanical ventilation.In the meanuhile,15 cases as control group with same standand.The results of the arterial blood gas analysis of the three groups before and after treatment show:one-way valve group had clear improvement in ventilation,increasing PaO 2 and decreasing PaCO 2 While in the other group,only increasing PaO 2,but no evident improvement in PaCO 2.In view of its non-traumatic characteristics,it can be practised clinically.Merits and demerits.Points for attention are discussed in the essay.
