摘 要:目的了解sCD23及CD23在系统性红斑狼疮(SLE)发病中的变化及意义。方法采用双抗体夹心ELISA法、SP及APAAP免疫组化法检测SLE患者血清sCD23水平、皮损及外周血单个核细胞(PBMC)CD23表达,对sCD23及CD23同ANA、抗dsDNA抗体关系进行了比较。结果①病人组血清sCD23水平〔(4.10±3.16)ng/ml〕与正常对照组〔(1.48±0.41)ng/ml〕比较显著增高(P<0.005),活动期水平明显高于静止期;②血清sCD23与ANA滴度、抗dsDNA抗体等具有明显的平行关系;③皮损表皮少数(3/9)弱表达CD23,PBMC则明显表达CD23。结论CD23及sCD23在SLE发病中有一定的意义,其主要作用可能与抗原处理和递呈以及促进B淋巴细胞分化、增殖产生自身抗体有关。Objective To elucidate the role of CD23 in the pathogenesis of SLE. Methods We detected the levels of sCD23 in sera and the expression of CD23 in cutaneous lesions and PBMC from SLE patients with ELISA and immunohistochemistry techniques, respectively, then analysed the relationship of CD23 system with the titers of ANA, anti dsDNA IgG. Results 1. The levels of sCD23 in patients were significantly higher than controls. Furthermore, in comparison to that in static stage, sCD23 of patients with active disease was markedly higher. 2. The correlationship between CD23 and ANA, anti dsDNA IgG was well positive. 3. The expression of CD23 was weak only in minor lesions (3/9), while strongly on the PBMC from all patients. Conclusions CD23 system may play key role in SLE, mainly the antigen presentation process, further promote the differentiation and proliferation of B lymphocytes and production of autoantibodies. The study reveals that CD23 may also be an indicator for monitoring the activity of SLE.
