手外伤后软组织缺损的皮瓣修复

摘　　要：目的:探讨手外伤后软组织缺损的外科皮瓣修复。方法:2007年2月至2008年1月,共治疗了88例手外伤后伴有软组织缺损的患者。根据术前软组织缺损的情况,分别采用了手指掌侧V-Y推进皮瓣10例,双三角皮瓣6例,指动脉逆行岛状皮瓣6例,手背筋膜逆行岛状皮瓣4例,手指背筋膜逆行岛状皮瓣8例,锁骨下皮瓣30例,腹股沟皮瓣10例,前臂皮瓣6例,邻指皮瓣4例,股前外侧皮瓣2例,拇趾甲皮瓣2例,进行皮瓣修复手部软组织缺损。结果:术后只有1例出现轻度感染,1例皮瓣部分坏死,经换药后愈合良好。2例腹股沟皮瓣断蒂后皮瓣雍肿,行去脂术后外形良好。结论:手部外伤后软组织缺损,根据软组织的不同缺损情况,选用恰当的皮瓣修复方法,具有创面皮直接覆盖,感染率低,保留手指长度,手部外形良好等优点。Objective：To discuss the flaps repair for soft tissue caused by hand injuries.Methods：From February 2007 to January 2008,88 cases injuries with soft tissue defect of hand were treated.According to different defect of hand soft tissue caused by hand injuries,different techniques were used,including finger V-Y advancing flaps（10 cases）,bilateral lateral triangular flags（6 cases）.finger artery reserve pedicle island flaps（6 cases）,dorsal metacarpal fascia reserve pedicle island flaps（4 cases）,dorsal fingeral fascia reserve pedicle island flaps（8 cases）,flaps under clavicle（30 cases）,pedicled groin flaps（10 cases）,crossed arm flaps（6 cases）,neighboring finger flaps（4 cases）,femoral flaps（2 cases）,wrap-arround flaps（2 cases）.Result：After the operation,there is one cases with flaps necrosis infection.There is one case with flaps part defect,which were removed afterwards.Only two cases had to have redundant of pedicled abdommopartes flaps cut off.Conclusion：Based on different sores of soft tissue defect,proper ways of primary emergency repair were chosen,which bring many advantages,such as covering the wound directly for the first time,low infection rate,remany the long of the fingers and satisfactory shape.
