中药洗剂治疗湿疹的系统评价

摘　　要：目的:系统分析中药洗剂治疗湿疹的有效性、安全性,以期做出客观、可信的评价,指导临床。方法:计算机检索VIP(1989-2008)、CHKD(1994-2008),纳入以湿疹患者为研究对象、比较中药洗剂与西药洗剂治疗效果的随机对照试验,评价纳入研究的质量,采用Jadad量表记分法,并用RevMan4.2.7进行Meta分析。结果:共纳入24个随机对照试验,包括3113例患者,Meta分析显示:(1)异质性检验:固定效应模型分析发现Q=42.68,P=0.003<0.05,表明研究间的效应量是异质的,因此改为用随机效应模型;(2)合并效应量OR合并为0.17,95%的置信区间为[0.11,0.29];(3)总体OR合并有统计学意义(P<0.05),说明中药洗剂治疗湿疹的疗效明显高于西药对照组。结论:中药洗剂治疗湿疹是一种有效的方法,且未发现较重的副作用。但由于存在发表偏倚和普遍低质量的随机对照试验,使结论的可信度降低。Objective： To systematic analyze the effectiveness and security of Chinese Lotions,to expect to review objectively and believablely,guiding clinic.Method： To search data in the VIP database（1989-2008）,CHKD（1994-2008）,and adopt the randomized control trial,with the eczema patients as the study subjects,for the comparison of the effects between the Chinese medicine lotions and the western medicine lotions.To evaluate the quality of the adoption study,and adopt the Scoring Method of Jadad Scale before processing the Meta analysis using the software of RevMan4.2.7.Results： 24 randomized control trials were adopted,including 3113 cases.（1） Heterogeneity Test： It was found through analysis with fixed effect model that Q=42.68,and P=0.0030.05,which indicates that the effect sizes between studies are heterogeneous,therefore,random effect model was adapted instead.（2） Total effect size＇ ORunited is 0.17,with the 95%confidence interval [0.11,0.29].（3） χ2= 6.91（P0.00001）,P0.05,the total ORunited has statistic significance,it indicates that the treatment of eczema with the Chi-nese Medicinal Lotions is obviously better than that of western medicine lotions.Conclusion： It is a effective method to treat eczema with Chinese Medicinal Lotions,and there is no serious ADR.However,because of publication bias and RCTs of universal lower quality,it makes the credibility lower.
