几种检测指标预测宫内感染的价值

摘　　要：目的比较几种快速预测宫内感染的方法，了解羊水中ＩＬ－６在宫内感染时的水平。方法取１９例宫内感染孕妇和８８例无宫内感染孕妇的血和羊水标本，分别检测血白细胞（ＷＢＣ）、Ｃ－反应蛋白（ＣＲＰ）和羊水中乳酸脱氢酶（ＬＤＨ）、葡萄糖（ＧＬＵ）和ＩＬ－６，羊水沉渣涂片革兰染色，胎盘进行病理学检查。结果宫内感染组的粒细胞比例、羊水中ＬＤＨ和ＩＬ－６水平显著高于对照组，羊水中ＧＬＵ显著低于对照组。根据各项与宫内感染有关的实验结果，将羊水ＧＬＵ≤１０ｍｍｏｌ／Ｌ、羊水ＬＤＨ≥３３０Ｕ／Ｌ、羊水ＩＬ－６≥１３００ｎｇ／Ｌ作为宫内感染的预测指标。结论羊水ＧＬＵ、ＬＤＨ和ＩＬ－６水平均对诊断宫内感染有一定帮助，特别是ＩＬ－６可较其它指标更敏感地预测宫内感染，能及时反映宫内情况，有一定的临床应用价值。Objective To investigate the level of interleukin-6 in women with intraamniotic infection(IAI),and to compare it with the value of several methods in the detection of IAI. Methods Blood and amniotic fluid weresampled from 19 women with IAI and 88 women without IAI.The blood analysis included white blood cell count and CRP.Amniotic fluid analysis included a Gram's stain,glucose,LDH and interleukin-6.Placentas were examined by pathology. Results The levels of LDH and IL-6 in amniotic fluid of women with IAI were higher than women without IAI,and the levels of GLU in amniotic fluid of women with IAI were less than those in control group.Glucos10mmol/L,LDH330U/L and IL-61300ng/L in amniotic fluid can identify IAI. Conclusions Detecting the levels of GLU,LDH and IL-6 in amniotic fluid could identify IAI,especially IL-6 in amniotic fluid is a sensitive sign for IAI.
