谷氨酰胺强化营养在重型颅脑损伤中的临床应用

摘　　要：目的 探讨早期丙氨酰谷氨酰胺（Gln）强化营养对重型颅脑损伤患者热休克蛋白70（HSP70）、血糖、蛋白质代谢的影响及对机体的保护作用.方法 重型颅脑损伤患者42例,随机分为Gln治疗组和对照组各21例,Gln组给常规营养支持和综合治疗外另给丙氨酰谷氨酰胺注射液,共7 d.比较治疗前后患者体内HSP70、血糖,ALB、NB变化.结果 Gln组治疗后HSP70显著高于治疗前和对照组（P〈0.01）,血糖,ALB、NB较对照组明显改善（P〈0.01）.结论 早期补充Gln能明显增加体内HSP70,可能有效减轻重型颅脑损伤患者应激状态.同时,可能有助于控制患者高血糖和改善低蛋白血症.Objective The study was to investigate the effect of glutaminc （Gin） enriched nutrition in early stage on heat shock protein 70 （ HSPT0）, blood glucose, and protein metabolism in patients with severe head injury and protective effect on the body. Methods 42 patients with severe head injury were randomly divided into Gln treatment group and control group,with 21cases ,respectively. Gln group were given the conventional nutritional support comprehensive treatment and Gin for a total of 7days in addition, We then compared the changes of HSP70, glucose,ALB, NB before and after treatment. Results After treatment of Gln, HSP70 was significantly higher than that before treatment and the control group （P 〈 0.01 ）. Besids blood glucose, ALB and NB were improved significantly compared with the control group（ P 〈 0.05 ）. Conclusion Early using Gin may effectively reduce the body＇s stress, shorten the acute stress period, make the patients to get in the period of stable and recovery as soon as possible. Meanwhile, the status of high blood glucose and hypoproteinemia may be improved.
