SLIPA喉罩对比普通喉罩影响气道通气的相关因素分析

摘　　要：目的观察分析SLIPA喉罩对比普通喉罩应用于乳腺外科手术围术期患者呼吸参数变化，术后并发症方面各个因素之间的相关性，掌握最佳的气道管理指征。方法乳腺外科手术患者250例，ASAI～II级，异丙酚、瑞芬太尼、顺式阿曲库铵全麻诱导后置入SLIPA喉罩或普通喉罩，纤支镜确定喉罩位置，记录术中呼出气量、气道顺应性、气道阻力变化，观察患者术后唾液分泌、咽痛、声嘶情况。结果SLIPA喉罩与普通喉罩相比，呼吸参数变化小，术后唾液分泌、咽痛、声嘶明显减少。SLIPA喉罩各变量之间比较，气道阻力、咽痛、声嘶、唾液分泌与喉罩偏移之间均存在显著的正相关关系；控制变量为呼出气量和气道顺应性时，喉罩置入次数与咽痛和唾液分泌之间存在显著的正相关关系，并且唾液分泌与喉罩置入次数和喉罩偏移之间存在显著的正相关关系。结论SLIPA喉罩对比普通喉罩呼吸参数影响小，术后并发症少。能提供最佳的气道通气指征。Objective To compare respiratory parameters and postoperative complications between SLIPA laryngeal mask and laryngeal mask airwan （LMA） in mammary gland operation . Methods Two hundred and fifty patients of ASAI-II underwent mammary gland surgery, using SLIPA LMA or LMA after general anesthesia with propofol, remifentanyl or cisatracurium. The LMA position was confirmed with bronchofibroscopy; airway expiration, airway compliance and airway resistance were recorded and counted; saliva secretion, pharyngodynia and trachyphonia after operation were observed. Results SLIPA LMA group was more stable than LMA in respiratory parameters. Postoperative incidences of saliva secretion, pharyngodynia and trachyphonia were significantly lower in SLIPA LMA group. Among respiratory parameters including airway resistance, saliva secretion, pharyngodynia and trachyphonia in SLIPA LMA there were positive correlations. When airway expiration and airway compliance were set as the control variables, times of SLIPA LMA were positively correlated with saliva secretion and pharyngodynia; and saliva secretion was positively correlated with times of SLIPA LMA. Conclusion Compared to LMA, SLIPA LMA can provide optimal airway ventilate in mammary gland surgery.
