慢乙肝患者血清肝纤维化与肝功能指标及PLT的相关研究

摘　　要：目的探讨慢乙肝患者血清肝纤维化指标与肝功能指标及血小板数量(PLT)的相关性。方法对慢乙肝患者肝功能指标和PLT测定的同时,采用增强化学发光免疫分析法测定血清透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型胶原(PCⅢ)、Ⅳ型胶原(CⅣ)四个肝纤维化指标,然后应用SPSS13.0统计分析软件对所得数据进行相关性分析。结果肝纤维化指标与谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、总胆汁酸(TBA)成正相关,与白蛋白(ALB)和PLT呈负相关;在肝炎分型各组中,随着肝炎严重程度的增大,HA的值明显增大(P<0.05),LN、PCⅢ、CⅣ变化不明显;随着PLT值的增大,肝纤维化四项指标中除了LN,HA、PCⅢ、CⅣ三项的血清水平均显著下降(P<0.05)。结论 HA与肝功能各指标及PLT的相关性最好,是反映肝纤维化最好的指标之一,综合考虑HA等相关肝纤和肝功能指标及PLT是诊断肝纤维化有效的血清学方法。Objective To investigate the relationship between the liver fibrosis indexes and the liver function indexes,PLT in patients with chronic hepatitis B disease. Methods The liver function indexes and PLT were measured,meanwhile,the values of the four liver fibrosis indexes,including HA,LN,PCⅢ and CⅣ were detected by the method of enhanced chemiluminescent immunoassay.Then the data were correlatively analyzed with the statistics software SPSS13.0. Results The liver fibrosis indexes were positively correlated with ALT,AST,TBIL,and TBA,but negatively correlated with ALB and PLT.As the hepatitis B becoming more serious among each group,the values of HA were increased significantly（P0.05）,but the values of LN,PCIII,and CⅣ were not changed significantly. With the increasing value of PLT,the values of HA,PCⅢ,and CⅣ were all decreased significantly（P0.05） except LN. Conclusions The value of HA has the best correlation with the liver function indexes and PLT,and it is one of the best liver fibrosis indexes.Combined considering the values of the liver fibrosis indexes and liver function indexes and PLT is a good way for predicting the degrees of liver fibrosis.
