摘  要:目的探讨彩色多普勒超声引导PICC的方法、可行性和优越性。方法将40例经传统方法穿刺失败或无法进行穿刺者在彩色多普勒超声引导下行PICC,并与40例用传统方法行PICC者在置管成功率及并发症等方面作一比较。结果传统方法穿刺组:首次成功率为72.5%(29/40);二次成功4例,三次成功2例,失败5例,总成功率为87.5%(35/40);超声引导组:首次穿刺成功38例,首次成功率为95%(38/40);二次成功2例,总成功率100%(40/40)。并发症:传统组并发症8例,其中穿刺局部血肿者7例、1例出现神经损伤;超声引导组并发症2例,均为穿刺局部血肿,无神经损伤。结论彩色多普勒超声引导下PICC可明显提高穿刺成功率,减少并发症的发生,同时还可较准确地判断导管尖端位置。Objective To discuss the method,feasibility and superiority of color Doppler ultrasound-guided PICC.Methods 40 cases who failed or can not be punctured by traditional methods had color Doppler ultrasound-guided PICC;40 cases used traditional methods for PICC(peripherally inserted central catheter),and to compare the success rate of puncture and complications of the two groups.Results Puncture group of traditional methods:the success rate of first puncture was 72.5%(29/40);four cases were successful after two times,two cases succeeded after three times,five cases failed,the total success rate was 87.5%(35/40).Ultrasound-guided group:38 cases succeeded at the first puncture,the success rate of first puncture was 95%(38/40);two cases succeeded after two times,the total success rate was 100%(40/40).Complications:eight cases of traditional group occurred complications.Seven cases were local hematoma,and one case was nerve damage.Two cases of ultrasound-guided group occurred complications.Both were local hematoma,no nerve damage.Conclusion Color Doppler ultrasound-guided PICC can significantly increase the success rate of puncture and reduce complications,and can also be more accurate to determine the location of catheter tip.
