摘  要:目的探讨慢性阻塞性肺疾病(COPD)患者血管内皮功能变化与COPD病情严重程度的相关性。方法选取2008年5月—2009年3月于仁济医院呼吸科就诊的COPD稳定期患者52例,分为轻度组(n=12)、中度组(n=12)、重度组(n=14)和极重度组(n=14);以26例健康志愿者作为对照组,分为健康吸烟对照组(n=13)和健康不吸烟对照组(n=13)。所有受试者接受肺功能检测,采用酶联免疫分析法(ELISA)和NO2-/NO3-硝酸还原酶法检测血浆内皮素-1(ET-1)、血管内皮生长因子(VEGF)和血清一氧化氮(NO)浓度。结果 COPD组血清NO和VEGF浓度均低于健康不吸烟对照组,差异有统计学意义(P<0.05);血清NO和VEGF浓度与第1秒用力呼气容积(FEV1)呈正相关(r=0.767,P<0.01;r=0.658,P<0.01)。COPD组血浆ET-1浓度均高于健康对照组,差异有统计学意义(P<0.05);血浆ET-1浓度与FEV1呈负相关(r=-0.816,P<0.01)。结论 COPD患者存在血管内皮功能紊乱,且与疾病的严重程度具有相关性。Objective To explore the relationship between vascular endothelial function and severity of chronic obstructive pulmonary disease (COPD).Methods Fifty-two patients with COPD in stable phase visiting Department of Respiratory Medicine,Renji Hospital from May 2008 to March 2009 were selected,and were divided into mild group (n=12),moderate group (n=12),severe group (n=14) and extremely severe group (n=14).Besides,another 26 healthy volunteers were served as control group,and were subdivided into healthy smoking control group (n=13) and healthy non-smoking control group (n=13).Pulmonary function tests were performed in all subjects.Concentrations of plasma endothelin-1 (ET-1),serum vascular endothelial growth factor (VEGF) and serum nitric oxide (NO) were detected by enzyme-linked immunosorbent assay (ELISA) and NO2-/NO3-nitrate reductase assay.Results The concentrations of serum NO and VEGF in COPD groups were significantly lower than those of healthy non-smoking control group (P0.05).The concentrations of serum NO and VEGF were positively related to forced expiratory volume in one second (FEV1) (r=0.767,P0.01;r=0.658,P0.01).The concentrations of plasma ET-1 in COPD groups were significantly higher than those in healthy control groups (P0.05).The concentration of plasma ET-1 was negatively related to FEV1 (r=-0.816,P0.01).Conclusion There exists vascular endothelial dysfunction in patients with COPD,which is closely related to disease severity.
