摘  要:目的探讨肺内病变穿刺活检中针胸膜夹角的影响因素及针胸膜夹角对气胸发生率的影响。资料与方法 828例患者在CT导引下穿刺活检,分别记录患者信息、病变情况、操作过程、并发症及处理、CT图像上测量的穿刺针与胸膜之间的最小角度及穿刺针调整角度等。将以上变量与针胸膜夹角及气胸发生率进行相关性分析。结果位于双肺上叶的病变可能影响针胸膜夹角,病变大小、穿刺点位置及进针深度与针胸膜夹角无确切相关性。针胸膜夹角(<75,°尤其<55°)与高的气胸发生率密切相关。结论针胸膜夹角及气胸发生率受多因素相互影响,病变部位可能影响针胸膜夹角,针胸膜夹角与气胸发生率密切相关。Objective To analysis the influence factors of needle-pleural angle and the correlation of needle-pleural angle with pneumothorax incidence during lung biopsy.Materials and Methods 828 patients underwent CT guided lung biopsy.The information of patients,lesions,biopsy processes,complications and treatment,were recorded.The minimal angle between needle and pleura and the adjustment angel were measured on CT images during biopsy.The correlation between needle-pleural angle and pneumothorax incidence was analyzed.Results The lesions located in the superior lobe may associated with small needle-pleural angle.The size of lesion,puncture site and needle path length had no correlation with needle-pleural angle.Needle-pleural angles(75°,especially55°) was associated with higher pneumothorax incidence.Conclusion The lesions location may affect the needle-pleural angle,which has correlation with pneumothorax incidence.
