摘  要:目的 探讨高压氧治疗对慢性硬膜下血肿钻孔引流术后脑复张的影响.方法 将215例慢性硬膜下血肿的患者分为两组.治疗组115例,在术后常规治疗的基础上行高压氧治疗.对照组100例,术后常规药物治疗.根据术后复查头颅CT血肿残腔最大层面的厚度(T),将脑复张的程度分为4级(Ⅰ级:T≤0.5 cm,Ⅱ级:0.5 cm<T≤1 cm,Ⅲ级:1 cm<T≤1.5 cm,Ⅳ级:T>1.5 cm).比较2个月内两组患者脑复张程度的差异.结果 1个月时两组脑复张程度差异有统计学意义(P<0.05),2个月时两组脑复张程度差异有统计学意义(P<0.01),治疗组高于对照组.结论 高压氧治疗能有效促进慢性硬膜下血肿术后脑复张,可作为慢性硬膜下血肿术后常规辅助治疗措施.Objective To discuss the effect of hyperbaric oxygen(HBO)in patients with postoperative brain's reexpansion of chronic subdural hematoma(CSDH). Method The clinical data of 215 patients with CSDH is divided into two groups. Treatment group:HBO therapy besides postoperative routine drug treatment 115 cases. Control group:postoperative routine drug treatment 100 cases. The degree of brain's reexpansion is divided into 4 grades according to the thickness(T)of hematoma residual cavity largest slice of postoperative cranial CT(Grade Ⅰ:T≤0. 5 cm, Grade Ⅱ:0. 5 cm 〈 T≤ 1 cm, Grade Ⅲ:1 cm 〈 T ≤ 1.5 cm, Grade Ⅳ:T 〉 1.5 cm). Contrast differences of two groups brain's reexpansion degrees in two months. Results The degree of brain's reexpansion had differences in postoperative first month(P 〈0. 05). The degree of brain's reexpansion had significant differences in postoperative second month(P 〈0. 01). The degree of brain's reexpansion in the treatment group was obviously higher than that in the control group. Conclusions HBO therapy can effectively promote postoperative brain's reexpansion of CSDH ,and it can be regarded as conventional postoperative adjuvant therapy.
