肺间质纤维化的高分辨率CT征象与肺功能检查的相关性分析

摘　　要：目的 探讨肺间质纤维化（PIF）的高分辨率CT（HRCT）征象与肺功能检查的相关性.方法 对61例有长期吸烟,慢性咳嗽的患者, 同时做肺功能及HRCT检查,并对患者HRCT的图像进行评估.然后与肺功能测定指标进行对照分析.结果 最大通气量（MVV）,第1秒用力呼出量（FEV1）和HRCT 5种征象相关（P〈0.01及P）.FEV1/用力肺活量（FEV1/FVC）和6种HRCT征象相关,其中与小叶间隔增厚,粗或细异常线影高度相关 与支气管血管束增粗,肺气肿,结节影为中度相关.结论 根据HRCT显示的肺间质纤维化的征象可以估计肺功能损害的轻重.Objective To study the correlation between radiographic signs on HRCT and pulmonary function test in pulmonary interstitial fibrosis（PIF）. Methods Sixty--one cases with smoking long--term and chronic cough underwent HRCT and pulmonary function tests. The correlation between HRCT findings and pulmonary function test was evaluated. Results The results showed that maximal voluntary ventilation（MVV）and forced expiratory volume in second （FEV1） were correlated with five signs of HRCT（P〈0.01 and P 〈0. 05）, respectively. FEV1/forced vital capacity （FVC） was correlated with six signs of HRCT, in which, interlobular septal, abnormal lines were marked, bronchiole expansion and bronchial vascular thickening, emphysema and nodules were middle. Conclusion Lung function in PIF can be evaluated with HRCT.
