小儿重症手足口病并发脑炎或脑干脑炎的MRI分析

摘　　要：目的探讨小儿重症手足口病并发脑炎或脑干脑炎的MRI表现。方法分析本院20082009年27例重症手足口病并发脑炎或脑干脑炎的MRI表现。结果27例患者中，15例累及脑干，其中3例同时累及颔髓，1例累及脊髓全长；累及枕叶8例，顶叶5例，额叶3例，丘脑2例。病变主要表现为T1WI稍低信号，T2WI稍高信号的斑片、斑点状影，边缘模糊，部分病灶在FLAIRT2WI序列上显示更为清楚。结论小儿重症手足口病并发脑炎或脑于脑炎的磁共振表现订一定的特点．町成为与其他感染性脑病鉴别的重要依据。Objective To study MR imaging findings of the severe hand--foot--mouth disease in children complicated by encephalitis or brain stem encephalitis. Methods MRI manifestations in 27 cases with severe hand--foot--mouth disease complicated by en cephalitis or brain stem encephalitis in our hospital between 2008--2009 were analysed. Results In 27 patients,the lesions involved the brain stem in 15 cases in which the spinal cord in one case and the whole spinal cord in 3 cases involved simultaneously and the occipital lobe in 8 eases, parietal lobe in 5 cases, frontal lobe in 3 cases and thalamus in 2 cases. The lesions appeared as spots or flake --like shadows, and slight hypointensity on T1WI, hyerintensity on T2 WI, some lesions were showed better on FLAIR T2 WI. Conclusion MRI findings of the severe had foot--mouth disease in children complicated by encephalitis or brain stem encephalitis are of certain characteristics, which can provide evidences for the differential diagnosis with the other infective encephalopathy.
