64层螺旋CT肝脏血管成像最佳条件的探讨

摘　　要：目的探讨64层螺旋CT在肝脏动脉、门静脉和肝静脉血管成像中的最佳触发阈值及延迟时间。方法将120例未患影响肝脏血流动力学改变的患者按触发阈值100HU、120HU、140HU、160HU、180HU、200HU分为6组，每组20人，分别对肝动脉行智能追踪技术（SureStart acquisition）法、门静脉及肝静脉期采用动脉扫描结束后间隔20s、40s时延迟成像法行上腹部的多期扫描，进行不同血管的三维重建并评价其效果。结果经统计学分析，当触发阈值为120HU肝动脉优良率最高（95％）；当时间间隔为20s时门静脉的优良率最高；当时间间隔为40s肝静脉的优良率最高。结论64层螺旋CT肝脏血管成像建议的扫描条件为肝动脉触发阈值120HU，肝动脉成像后间隔20s及40s扫描，对肝动脉、门静脉及肝静脉成像可达到较高的满意度。Objective To study the optimum parameters including the triggering threshold and delay time at 64--slice spiral CT hepatic angiography. Methods 120 patients with no any diseases affecting hepatic blood flow were involved in this study. 120 cases were divided into 6 groups according to different threshold（ 100 HU, 120 HU, 140 HU, 160 HU, 180 HU and 200 HU, each group in 20 cases） during hepatic CT angiography with SureStart acquisition technique. The.hepatlc portal vein （HPV） and the hepatic vein （HV） scan were begun at 20 s and 40 s after hepatic arterial （HA） scan. CT data of HA, HPV and HV were reconstructed with maximum intensity projection（MIP） and volume rendering （VR） technique and the image quality at different threshold and delay time was compared. Results The statistical analysis showed that the image quality in excellent and good was best when triggering thresh- old at 120 HU（95%） and delay time at 20 s and 40 s respectively. Conclusion At 64--slice hepatic CT angiography,the triggering threshold of HA,the delay time at 20 s and 40 s were recommended for HA,HPV and HV.
