摘 要:目的:探讨早期生长反应因子(Egr)-1与乙酰肝素酶(heparanase)蛋白在膀胱移行细胞癌(BTCC)中的表达情况及相关性。方法:采用免疫组织化学SABC法检测58例BTCC组织及10例正常膀胱黏膜组织的Egr-1蛋白、heparanase的表达情况,并分析两者表达情况与BTCC临床病理特征的关系,以及两者在BTCC中表达的相关性。结果:Egr-1蛋白和heparanase蛋白在10例正常膀胱黏膜组织中不表达,在BTCC组织中阳性表达率分别为70.69%(41/58)和58.62%(34/58)。Egr-1蛋白及heparanase蛋白阳性表达率在不同年龄、性别、肿瘤大小及数目的患者之间差异无统计学意义(P>0.05);不同病理分级及临床分期患者Egr-1蛋白及heparanase蛋白表达率差别有统计学意义(P<0.01)。在BTCC中Egr-1蛋白与heparanase蛋白表达呈正相关(r=0.3875,P<0.01)。结论:Egr-1及heparanase蛋白的高表达在BTCC的发生、发展中起重要作用,与非肌层浸润型膀胱癌进展为肌层浸润型膀胱癌过程相关。Objective:To investigate the relationship between the expression of Egr-1 protein and heparanase in bladder transitional cell carcinoma(BTCC).Methods:The expressions of Egr-1 protein and heparanase were examined by SABC(StreptAvidin-Biotin Complex)immunohistochemistry in 58 paraffin-embedded tissue specimens and 10 control of normal bladder tissues.The expression and relationship of Egr-1 protein and heparanase protein with clinicopathologic factors were analysed.Results:The positive expression rate of Egr-1 protein was 0 in normal bladder tissue,while it was statistically significant in BTCC(P 0.01).There was no expression of heparanase in 10 normal bladder tissues,while the expression of heparanase was detected in 34 of 58 BTCC(58.62%).The positive expressions of Egr-1 protein and heparanase were not significantly different in age,gender,tumor size and tumor quantity of BTCC(P 0.05),but they were closely correlated with the degrees of histological grades and clinical stages(P 0.01).The expression of Egr-1 protein was closely correlated with that of heparanase(r = 0.387 5,P 0.01).Conclusion:The positive expression of Egr-1 protein and heparanase might be the reference index in the development and invasion of bladder carcinoma.The results showed that Egr-1 protein may provide theoretical foundation of chemopreventive strategy for bladder cancer in the future.
