颈前路内固定治疗Hangman骨折的手术疗效

摘　　要：目的探讨Hangman骨折颈前路减压融合内固定的手术疗效。方法对8例Hangman骨折行颈前路颈2～3椎间盘切除减压植骨内固定,术后颈托外固定3月。结果 8例均获得了随访,时间6～24个月,患者术后3个月均获得了骨性融合,其中1例出现颈2椎体固定螺钉轻度拔出,无出现椎体移位成角畸形,无切口感染和神经损伤加重等其它并发症,不全瘫患者均有不同程度的恢复。结论颈前路颈2～3椎间盘切除减压植骨内固定治疗Hangman骨折,内固定可靠,能有效维等理想的复位。Objective To evaluate the effi cacy of anterior fusion and internal fi xation in the treatment of Hangman＇s fractures.Methods The eight cases of Hangman＇s fractures were treated surgically by anterior plating of C2,3 with reduction and interbody fusion.Cervical collar was used for three months after operation.Results All of the eight patients were followed up for 6-24 months.All cases achieved bony fusion within three months postoperatively.Only one screw of C2 vertebral was found pulled out lightly.The cases showed no anterior displacement or kyphosis,infection of incision,neurological lesion and other postoperative complications.The cases with incomplete spine cord injury attained improvement at different degrees.Conclusion C2,3 anterior fusion and plate fixation can maintain excellent reduction in the treatment of Hangman＇s fractures.
