类风湿关节炎的治疗与I型胶原吡啶交联终肽的关系

摘　　要：目的探讨I型胶原吡啶交联终肽(ICTP)在类风湿关节炎(RA)患者联合给药治疗前后血清中的变化,并研究ICTP检测在RA中的初步应用。方法选择RA患者89例,给予甲氨蝶呤、柳氮磺吡啶和羟氯喹联合给药,于第0、6月留取患者血清。应用放射免疫法测定患者血清中ICTP水平,同时测定I型前胶原氨基端前肽(PINP)、血沉(ESR)和类风湿因子(RF)的值,并记录RA患者的疾病活动指数28(DAS28)。结果治疗后RA患者血中的ICTP、ESR、RF及DAS28均有明显下降,且ICTP与PINP、DAS28呈明显相关(r=0.12,P=0.04与r=0.29,P=0.03)。结论 RA患者血清中ICTP水平随治疗明显变化,提示血清中ICTP检测对RA的诊断及判断治疗情况有一定意义。Objective To study the preliminary clinical application and the effects of treatment on C-terminal cross-linking telopeptide o type I collagen （ICTP） in the serum of patients with rheumatoid arthritis （RA）.Methods 89 patients were treated with methotrexate,sulfasalazine and hydroxychloroquine.The serum level of ICTP was determined in those patients by radioimmunoassay,and the N-terminal telopeptide of type collagen（PINP）erythrocyte sedimentation rate（ESR）,rheumatoid factor（RF） and disease activity score 28 were also recored at baseline and at months 6 Results Serum ICTP,PINP,ESR,RF and DAS28 levels of RA were signf ically reduced after the therapy.Moreover,the levels of ICTP were positively correlated with the PINP and DAS28 （r=0.12,P=0.04 and r=0.29,P=0.03 respectively）.Conclusion The present data suggests that the levels of ICTP was changed with the therapy.It was means that the serum level of ICTP in the patients with RA can reflect the progression of the disease.
