布托啡诺联合丙泊酚靶控输注在老年患者结肠镜检查中的应用

摘　　要：目的研究布托啡诺与丙泊酚靶控输注用于老年患者的可行性和安全性。方法将60例接受肠镜检查的老年患者随机分为2组:B组,布托啡诺(10μg/kg)与异丙酚联合组:F组,芬太尼(1μg/kg)与丙泊酚联合组。分别记录并分析入室基础值,注射药物后,检查中,检查结束不同时点的呼吸频率(RR)、收缩压(SBP)、心率(HR)和氧饱和度(SpO2),意识消失时间,唤醒时间,丙泊酚用量,检查中病人反应情况,体动次数以及不良反应等。结果两组患者注射药物后SBP、HR、RR、SpO2下降,与基础值比较有统计学意义(P<0.05),其中F组RR下降明显,与B组比较有统计学差异(P<0.05);两组丙泊酚用量、意识消失时间、唤醒时间和检查中体动次数无统计学差异;B组患者头晕、恶心、呕吐、呼吸抑制等不良反应少于F组(P<0.05)。结论布托啡诺联合丙泊酚靶控输注用于老年患者肠镜检查安全有效。Objective To compare the effects and safety between butorphanol and fentanyl during target-controlled infusion of propofol in old patients undergoing colonoscopy.Methods 60 old patients（60-85 years old） undergoing eteroscopy were randomly allocated to two groups with 30 cases each.Anesthesia was maintained with premedication of butorphano（10 μg/kg,B proup） or fentanyl（1 μg/kg,F group） and target-controlled infusion of propofol.The time of induction,colonoscopy,recovery and total examination,the dosage of induction and total dosage of propofol,and HR,RR,MAP,SpO2 and adverse events were recorded.Results HR,RR,MAP,SpO2 in two groups decreased after infusions（P〈0.05）,RR of F group decreased significantly compared with B group（P〉0.05）.There were no significant difference in induction and total dosage.The times of induction and recovery between two groups were different（P〈0.05）.The adverse events rate in B group was lower than that in F group（P0.05）.Conclusion Butorphanol conbined with target-controlled infusion of propofol is a safe and effective anesthesia for old patients undergoing eteroscopy.
