PFNA治疗老年股骨粗隆间骨折疗效分析

摘　　要：[目的]总结应用防旋型股骨近端髓内钉(PFNA)治疗老年股骨转子间骨折的临床疗效。[方法]2005年6月～20010年4月采用PFNA治疗老年股骨转子间骨折52例,均在充分术前准备,患者能够耐受手术的前提下早期手术,牵引床闭合牵引复位,C臂动态透视,导针和瞄准器辅助,小切口植入PFNA内固定器械。[结果]所有患者均获得随访,随访时间3～24个月,骨折均愈合。功能恢复按Harris髋关节评分,优28例,良20例,可3例,差1例,优良率为92.3%。本组病例未出现股骨干骨折、股骨头坏死或切割、内植物断裂失败等并发症,无围手术期死亡病例。[结论]PFNA内固定技术治疗老年股骨转子间骨折具有很好的生物力学适应性,能缩短手术时间,减少术中出血量,降低术后并发症,疗效可靠,是治疗老年股骨粗隆间骨折的理想选择之一。[Objective] To generalize the curative and clinical effect of proximal femoral nail anti-rotation（PFNA） in the aged intertrochanteric fractures. [Methods] From June,2005 to April,2010,twenty six patients with intertrochanteric fractures were included in the study. All the patients were prepared fully. Once the conditions were mature,the operations were carried out as early as possible through minimally invasive surgery. [Results] All the cases were followed up from three months to twenty four months. All fractures healed according to Harris criterion,the results of 28 excellent and 20 good cases were obtained. The excellent and good rate of the function of the hip joint was 92.3%. No severe complications were found. [Conclusion] PFNA while applied to intertrochanteric fractures in the elderly has excellent biological and physical adaptation. It could shorten the operation time,reduce intraoperative blood,and reduce the complication. It is the ideal selection for treating intertrochanteric fractures in the elderly.
