摘　　要：目的了解和评价乳酸脱氢酶及总蛋白定量对腹水性质的诊断价值。方法采用7600全自动生化分析仪检测266例腹水患者血清和腹水乳酸脱氢酶及总蛋白水平,以传统的腹水分类法中漏出液与渗出液的诊断依据作为标准,提出对腹水性质诊断的最佳界值。结果腹水总蛋白/血清总蛋白、腹水LDH、腹水LDH/血清LDH的最佳临界值分别为0.3、74U/L和0.4,大于或等于上述界值者支持渗出性腹水,小于上述界值者支持漏出性腹水。结论腹水/血清总蛋白比值、腹水LDH、腹水/血清LDH比值对于鉴别腹水为漏出液或是渗出液有较大意义,可以作为传统的腹水性质分类法的补充。Objective To evaluate the diagnostic value of lactate dehydrogenase and total protein level for ascites nature.Methods Lactate dehydrogenase and total protein level in 264 patients＇ serum and ascites were detected by 7600 automatic biochemistry analyzer.Optimal delimitation of transudate and exudate were suggested,according to the standard of traditional ascites nature classification.Results Optimal delimitation for ratio of ascites total protein and serum total protein,ascites lactate dehydrogenase level,ratio of ascites lactate dehydrogenase level and serum lactate dehydrogenase level was 0.3,74 U/L and 0.4,respectively.Exudate was supported if the number was larger or equal,and vice versa.Conclusion Ratio of ascites total protein and serum total protein,ascites lactate dehydrogenase level,ratio of ascites lactate dehydrogenase level and serum lactate dehydrogenase level have significance in identification of transudate and exudate.They can be complementation of traditional ascites nature classification.
