摘 要:目的探讨经皮二尖瓣球囊成形术(PBMV)前后血气变化,分析血气变化是否可用于PBMV术后疗效监测的指标。方法采用改良Inoue单球囊技术行PBMV,术前一天及术后三天以瑞士AVL995型全自动血气分析仪测定患者动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、肺泡气-动脉血氧分压差(P(A-a)O2)等指标。结果PBMV术后患者血液动力学显著改善,PaO2由术前9.41±0.72kPa上升至术后10.00±0.85kPa(P<0.01),P(A-a)O2由2.60±0.78kPa降至术后的1.77±0.75kPa(P<0.001)。结论血气分析的改善也可作为检测重症二尖瓣狭窄(MS)患者PBMV手术疗效的参考指标。Objectives To explore the changes of blood gas before and after percutaneous blloon mitral vavuloplasty,analize whether the changes of blood gas can be used for curative effect observation after PBMV.Methods The operation of PBMV was performed with improved Inoue technique in 16 patients with rhumatic severe mitral stenosis(MS).The patients′ arterial partial pressure of oxygen (PaO 2),arterial partial pressure of carbon dioxide (PaCO 2) alveolararterial oxygen difference (P(A-a)O 2) were determined by AVL-995 Automatic blood gas system (made in Switzerland) one day before the operation and three days after the operation. Results Most patients had a significant improvement of hemodynamics after the operation.the PaO 2 was increased from 9.41±0.72 to 10.00±0.85kPa ( P <0.01),P(A-a)O 2 was decreased from 2.60±0.78 to 1.77±0.75kPa ( P <0.005).Conclusions The improvement of blood gas can be reference target for determine curative effect of patients after the operation of PBMV.
