摘 要:目的探讨PET/CT联合MSCT的多平面重建(MPR)对孤立性肺结节良恶性鉴别诊断的价值。方法 43例孤立性肺结节患者行^(18)F-FDG PET/CT显像和MSCT扫描后进行多平面重建,^(18)F-FDG PET/CT用目测法结合半定量分析法判断良恶性,MSCT的MPR则根据病灶形态学特征判断良恶性,所得^(18)F-FDGPET/CT结果、^(18)F-FDG PET/CT联合MSCT的MPR的结果分别与病理检查结果对照比较。结果 43例SPN中病理及随访证实26例恶性,17例良性。26例恶性SPN中PET/CT显像阳性23例,阴性3例;17例良性SPN中,PET/CT显像阳性7例,阴性10例。^(18)F-FDG PET/CT对恶性结节诊断的敏感性为88.5%,特异性为58.9%,准确性为76.8%;联合MSCT的MPR,对恶性结节诊断的敏感性为96.2%,特异性为76.5%,准确性为88.4%。结论 ^(18)F-FDG PET/CT联合MSCT的MPR可以提高SPN良恶性鉴别诊断的准确性。Objective To investigate the clinical value of the ^18 F-fluorodeoxyglucose (FDG) PET/CT combined with MPR of MSCT to differentiate solitary pulmonary nodules (SPN). Methods 43 patients with SPN were examined with ^18F-FDG PET/CT and MSCT, MPR images were obtained on workstation. The differential diagnosis of SPN were made using visual method and semi-quantitative method on PET/CT and morphological characteristics on MSCT. The results of ^18F-FDG PET/CT and 18F-FDG PET/CT combined with MPR of MSCT were compared with pathological results. Results Of the 43 SPN, 26 were malignant and 17 benign were proved pathologically or by follow-up. 23 SPN were positive on PET/CT among 26 malignant SPN with 3 negative; 7 SPN were positive on PET/ CT among 17 benign SPN with 10 negative. The sensitivity, specificity and accuracy of 18F-FDG PET/CT was respectively 88.5% ,58.9% ,76.8%. 18FDG-PET/CT combined with MPR of MSCT :96.2% ,76.5% ,88.4%. Conclusion ^18F-PET/CT combined with MPR of MSCT can improve the accuracy of the diagnosis of differentiating malignant SPN from benign.
