T管加垫圈堵塞法治疗唇状肠瘘(附4例临床治疗体会)

摘　　要：目的观察使用Ｔ管加垫圈堵塞肠瘘的临床效果。方法选用外径与瘘口相似的乳胶Ｔ管。取直径约３ｃｍ圆形橡皮片，在其中央剪一圆孔，孔径略小于Ｔ管外径，将Ｔ管长臂穿过该孔，直至橡皮片（垫圈）与Ｔ管短臂紧贴。适当剪去部分短臂两端和前壁，将短臂连同垫圈通过瘘口插入肠腔，然后轻拉Ｔ管长臂，使垫圈紧贴瘘口，将长臂固定于正常皮肤并接上消毒塑料袋。结果本组４例使用本法，均有理想堵塞效果，２例免除再手术，２例用此法为根治手术创造了良好条件。结论本法对肉眼可见粘膜外翻的肠瘘具有显著的治疗作用。Objective To observe the clinical effectiveness of T tube combined with rubber washer used to obstruct the hole of intestinal fistula.Methods T tube is pulled through the central hole of a flake of rubber(inner tire of bicycle wheel)until both short arms of T tube are in close contact with the flake.T tube short arms are appropriately cut and pushed together with the washer into intestinal lumen through the fistula hole.Slight traction is exerted to T tube which is then attached to normal skin and connected to a plastic bag.Results Obstructing effect was very ideal in all the 4 cases:Re-operation was avoided in 2 cases;Good condition was created for radical operation in the other 2 cases.Conclusion The method presented is very useful in the treatment of intestinal fistula with mucosa visibly everted.
