摘  要:目的探讨COPD患者进行6分钟步行试验(6MWT)前后血气变化及其与肺功能的关系。方法选择46例COPD患者,另选经肺功能检查证实肺功能正常的43名其他患者作为对照组,进行6分钟步行试验,试验前后记录患者的血压、心率及呼吸频率,采用美国GEMPremier3000型血气分析仪测定血气结果,所有患者均测定肺功能。结果试验组和对照组在6分钟步行试验距离、动脉血氧饱和度下降分数、血乳酸浓度方面,差异均具有显著性,6分钟步行距离同FEV1、FEV1/FVC正相关,动脉血氧饱和度下降分数同FEV1、FEV1/FVC比值呈明显负相关(P<0.05)。结论 6分钟步行试验能对COPD患者的肺功能及全身功能状态进行评估,价格低廉,方便,更适合在基层医疗单位推广。Objective To investigate the association between the changes of arterial blood gas and lung function in patients with chronic obstructive pulmonary disease in 6-minute walk test.Methods 46 patients with chronic obstructive pulmonary disease were divided into experimental group,43 other patients with normal lung function as a control group.Blood pressure,heart rates and respiratory rates were recorded before and after testing, arterial blood gas were measured by the U.S. GEM Premier3000 blood gas analyzer, lung function were measured in all patients.Results Comparing to the control group,there were significantly differences in the 6-minute walk distance, degression fraction of arterial oxygen saturation, blood lactate concentration. The 6-minutes walk distance were positively correlated with FEV1 and FEV1/FVC, degression fraction of arterial oxygen saturation were negatively correlated with FEV1 and FEV1/FVC(P0.05).Conclusions 6-minute walk test (6MWT) can assess lung function and the body function of the patients with chronic obstructive pulmonary disease.this test is simple to perform,inexpensive and convenient.it is more suitable for primary health care units.
