右室双腔心23例的外科诊治

摘　　要：目的总结右室双腔心的诊断及外科治疗经验。方法回顾性分析右室双腔心23例患者的诊治情况,其中单纯右室双腔心1例,同时合并其它心内畸形22例。23例患者中肌束型12例,肌隔型7例,混合型4例。所有患者均予切除右心室异常肌束,同期予室间隔缺损直接缝合19例,自体心包加宽右室流出道11例,矫正合并的其它心内畸形3例。结果 23例患者中均痊愈出院,无近期、远期死亡及相关并发症。结论右室双腔心常合并其它心内畸形,外科手术矫治心内畸形是唯一的根治方法。熟练掌握双腔右心室的病理解剖、正确识别异常肌束及有效的右心室流出道疏通是手术成功的关键。Objective To summarize the experience in the diagnosis and surgical treatment of double-chambered right ventricle（DCRV）.Methods Twenty-three patients with DCRV proved by operation were retrospectively analyzed,including 7 female and 16 male aged 3～29,1 case of pure DCRV and 22 cases of DCRV combined with other intracardiac anomalies.Results All patients were cured by surgery.No death and complication occurred.Conclusion DCRV is usually found to be complicated with other intracardiac anomalies.Surgery is the only method to cure this disease.It is critical to perfectly grasp DCRV pathoanatomy,correctly identify abnormal muscle bundle and effectively clear out right ventricle outflow tract during operation.
