摘 要:目的研究电环切除术对组织的热损伤及组织学诊断的可信性。方法121例电环切除术,其中89例为病灶电环切除,32例为病灶电环活检,观察切出标本切缘的热损伤程度及病理学诊断。结果603%(73/121)标本的切缘可见不同程度的热损伤改变,表现为碳化和(或)凝固。碳化区位于最外层,较薄;凝固区在碳化区旁,表现为细胞错乱,数层细胞紧密单向性倾斜排列,细胞拉长,着色加深,胞核轻微扭曲增大,染色质模糊不清似烟雾状。细胞膜完整,上皮组织结构完好。热损伤程度与电压、电环钢丝横截面直径及电环移动速度有关。结论电环切除术对组织热损伤少,切出组织可供病理诊断,操作简便,止血好,性能安全,值得推广。Objective\ To study thermal damage of tissues obtained by loop eletrosurgical excision procedure(LEEP)and its reliability in histopathological diagnosis.Methods\ Thermal damage of the tissue margins and pathological diagnosis were studied in 121 cases,89 of which for LEEP excision,the other 32 for LEEP biopsy.Results\ 60 3%(73/121)of specimen's margins had thermal damage to varying degrees,presenting carbonization or coagulation zone.The carbonization zone was located at the outmost layer and was very thin.The coagulation zone was adjacent to the carbonization zone,and showed cell distortion,dense and tilt arrangement of endocervical cells,elongation of cells,deepening in stain,mild enlargement and twist of the nuclei,and smudgy of nuclear chromatin.The cell membranes were intact,and the tissue structure was well preserved.The degree of thermal damage was related to voltage,diameter of the loop wire and moving velocity of loop during operation.Conclusions\ The thermal damage of tissues was mild in LEEP.The tissue samples by LEEP are suitable for histopathological examination.LEEP was easy to operate,efficient in hemostasis,and safe in performance.It is worthy of popularizing.
