摘 要:目的:探讨成都地区社区居民高尿酸血症和糖脂代谢紊乱之间的关系。方法:利用成都地区慢性病防治项目的现况调查资料,分析年龄40岁及40岁以上中老年人群的资料,比较高尿酸血症与正常尿酸人群的糖脂代谢情况,并进一步分析高尿酸血症与相关危险因素的关系。结果:高尿酸血症组空腹血糖受损、单纯性糖耐量异常、空腹血糖受损合并糖耐量异常、糖尿病的患病率均明显高于尿酸正常组(P<0.05);高尿酸血症组高胆固醇血症、高甘油三酯血症、高LDL-C和低HDL-C的患病率明显高于尿酸正常组(P<0.05);Logistic回归分析显示,男性、老龄、肥胖、血压升高、高血糖、胆固醇和甘油三酯升高都是高尿酸血症的独立危险因素(P<0.05),女性和HDL升高是高尿酸血症的保护性因素,其中年龄和性别是不可控制的因素。结论:高尿酸血与糖脂代谢关系密切,是糖尿病和高脂血症发生发展的重要危险因素。成都地区社区中老年人群高尿酸血症和糖脂代谢紊乱的患病率很高,高尿酸血症人群更易发生糖尿病和血脂紊乱。尽早对社区人群进行高尿酸血症和糖脂代谢干预,对降低心血管疾病事件的发生和疾病负担具有重要意义。Objective:To study the relationship between the hyperuricemia(HUA)and disorders of glucose and lipid metabolism among the community-dwelling residents in Chengdu area.Methods:A cross-sectional population-based survey for chronic diseases in Chengdu was performed in 2008.By analyzing the data of population aged 40 years and over,we compared the glucose and lipid metabolism between normal uric acid and HUA,and further analyzed the relation between HUA and its related risk factors.Results:The prevalence of impaired fasting glucose,simple glucose tolerance,impaired fasting glucose combined glucose tolerance and diabetes was significantly higher in HUA individuals than normal uric acid individuals(P0.05).Compared with normal serum uric acid,the prevalence of dyslipidemia including hypercholesterolemia,hypertriglyceridemia,high LDL-C and low HDL-C were significantly higher(P0.05).Logistic regression analysis showed that male,aging,obesity,high blood pressure,hyperglycemia,elevated triglycerides and total cholesterol are independent risk factors for HUA(P0.05);female and elevated high density lipoprotein cholesterol are protective factors for HUA.Both age and sex are nonmodifiable risk factors.Conclusions:HUA is closely correlated with glucose and lipid metabolism.People with HUA are more susceptible to diabetes and dyslipidemia.There is a great significance to reducing the incidence of cardiovascular disease and disease burden by intervening HUA and its related risk factors in the community-dwelling people as soon as possible.
